This Release of Liability is just a template, and as is typical of US law, it is just a social deterrent from people suing you, they can still sue you regardless if they sign this or not. If you feel you need this for any reason, you should also consider getting some insurance.

Release of Liability for Transportation to UCA Cheerleading Camp in Panama City Beach, FL

I, and my heirs, in consideration of my participation in the UCA Cheer Camp on July 30-Aug. 2 at Edgewater Beach Resort, hereby release Shelby County Board of Education and, its officers, employees and agents, and any other people officially connected with this event, from any and all liability for damage to or loss of personal property, sickness or injury from whatever source, legal entanglements, imprisonment, death, or loss of money, which might occur while participating in this event. Specifically, I release said persons from any liability or responsibility for Transportation to and from UCA Cheer Camp. I am aware of the risks of participation. I hereby state that I am in sufficient physical condition to accept a rigorous level of physical activity. I understand that the driver of the private vehicle being used for transportation does not provide medical coverage for me. I verify that I will be responsible for any medical costs I incur as a result of my participation.
